State your full name in English exactly as written in your passport. You will not be allowed to travel to the UK if all the names you enter in English do not exactly match what is in your passport. Include all initials and any punctuation, for example dashes or apostrophes. Refer to examples below.

Select your gender from the list of options. This should be your sex as stated in your passport at time of notification.

Passport Number:

You must be an Emirati, Kuwaiti, Omani, Bahrain, Saudi Arabia or Qatari passport holder to travel under the EVW scheme. The number should be entered exactly as shown in your passport including any letters. Pay particular attention to the letter O and the number 0; and the letter I and the number 1. Special Characters are not permitted. Refer to examples below.

Issue Date:

Select the date on which the passport was issued, indicating the day, month and year as it appears in your passport. Exactly as recorded in your passport. In DD/MM/YYYY format. Refer to examples below.

Expiry Date:

Select the expiry date of your passport indicating the day, month and year exactly as recorded in your passport. In DD/MM/YYYY format. It is your responsibility to ensure that your passport is valid for the duration of your visit to the UK. Refer to example below.

Issue Authority/ Place of Issue:

State the location where your passport was issued and exactly as indicated on the data page of your passport. Please include both the city and the country that issued the passport. Refer to examples below.

Country of Birth:

Select your country of birth exactly as it appears in your passport. Refer to the examples below.

Place of birth:

Enter your place of birth as written in your passport. This must be the village, town, city and state, province in which you were born, exactly as stated in your passport. See examples below.

Departure Date:

Select the date of your departure from your country to the UK. This information is reflected on your travel itinerary and travel tickets. This information is required for your EVW Application.

It is the date and time of departure for your journey that will end in the UK. If you are taking a multi-leg journey please provide details of the last leg to the UK. For example, if you are flying from Doha to Dubai then Dubai to London then the local date and time of departure would be for the last leg from Dubai to London.

This date must be the exact date you will travel as your EVW document will only be valid on the date selected. If you change your travel plans you will need to complete another form at least 48 hours before travel to the UK. The date should be in DD MM YYYY format. If you wish to apply for EVW for another journey to the UK in the next 3 months you will be able to do so once this form is completed.

Departure Location:

Select the airport, train station, port where you will be leaving from to enter into the UK. This information is reflected on your travel itinerary and travel tickets. If your travel includes multiple countries, list the last location from which you are directly entering the UK. This information is required for your EVW Application. This is the point of departure where you will leave for the UK. If you are taking a multi-leg journey please provide details of the last leg to the UK. Select the date you will be first arriving into the UK. Provide the scheduled arrival date for the flight, train or ship that you will be disembarking from in the UK. If you are travelling by private flight or ship, please provide the approximate time and date you intend to arrive. Date in DD/MM/YYYY format and time in 24 hour format. This information is reflected on your travel itinerary and travel tickets. If your travel includes multiple trips, list the first date of entry into the UK. Please note that you cannot apply for your EVW more than 3 months before your intended arrival date. This information is required for your EVW Application.

What is the main reason for your visit to the UK?

Select the main reason for your visit to the UK. EVW is only available for visits of up to six months. You will need to apply for a visa if you intend to stay in the UK for more than six months. The EVW scheme covers all types of visits for less than six months except those visiting for the purposes of marriage or to enter into a civil partnership, in which case they will need to apply for the appropriate visit visa. The Visiting the UK section describes the different categories of visitor, and what visitors in each category can and cannot do while they are in the UK.

Electronic Visa Waiver (EVW) holders can enter the UK with a valid EVW for the following purposes. This information is required for your EVW Application.

Business: A visitor may; attend meetings, conferences, seminars, interviews; give a one-off or short series of talks and speeches provided these are not organised as commercial events and will not make a profit for the organiser; negotiate and sign deals and contracts; attend trade fairs, for promotional work only, provided the visitor is not directly selling; carry out site visits and inspections; gather information for their employment overseas; be briefed on the requirements of a UK based customer, provided any work for the customer is done outside of the UK.

Tourism: A visitor may visit friends and family and or come to the UK for a holiday. A visitor may undertake incidental volunteering (i.e. the main purpose of the visit is not to volunteer), provided it lasts no more than 30 days in total and is for a charity that is registered with either the Charity Commission for England and Wales; the Charity Commission for Northern Ireland; or the Office of the Scottish Charity Regulator.

Study: Visitors may carry out the following study; educational exchanges or visits with a UK state or independent school; or a maximum of 30 days study, provided that the main purpose of the visit is not to study: (i) recreational courses (not English language training); (ii) a short-course (which includes English language training) at an accredited institution.

Medical Care: An individual may receive private medical treatment provided they meet the additional eligibility requirements. An individual may act as an organ donor or be assessed as a potential organ donor to an identified recipient in the United Kingdom, provided they meet the additional eligibility requirements.
